Management of Recalcitrant case of Oral Pemphigus with Intralesional corticosteroids – A case report.

Abstract

Background: Oral pemphigus is one of the prevalent autoimmune oral mucosal diseases, but the recalcitrant variant is infrequent and challenging to manage. 

Case description:  This report is about a female patient aged 46 years with long standing ulcers in oral cavity involving multiple sites. She was treated by a dermatologist with conventional corticosteroids and cyclophosphamide for six months. Patient had no relief and developed clinical signs of osteoporosis. She was referred to dental specialist for management. The patient was treated with intralesional corticosteroid injection (ICSI) and within 3 months, resulted in disease-free state. There was no recurrence in the follow-up period of one year.

Conclusion: Intralesional carticosteroid injection has advantages of high concentration of drug with long duration of action at the lesion site and no systemic adverse effects comparing to the topical and oral corticosteroids. Hence, ICSI need to be valued in treating chronic mucocutaneous lesion for patients benefit.
